Water Service Application


Water Service Agreement

CITY will furnish water service to the CUSTOMER for domestic use and consumption. Water service is to be supplied to CUSTOMER at the above address. CUSTOMER agrees to pay the CITY an application fee of $36. It will be billed to you on your first statement. CUSTOMER agrees to pay any fees, charges or rates as they are imposed by the CITY for CUSTOMER’s water service as they exist now or may be adjusted in the future. CUSTOMER agrees and understands the following billing schedule information: Water service bills are generated on or about the 1st day of each calendar month. The bills will reflect total consumption, meter charges, and other fees including the City of North Plains Transportation Utility Fee. The full balance of a water service bill is due by the 20th of the month. Failure to pay the balance in full by the end of the business day on the 20th will result in a past due balance and 10% late fee assessment. If a past due balance ages more than 40 days, the CITY will issue a 72-hour Notice and a fee will be assessed on the past due balance. If the past due balance is not paid in full by the end of the 43rd day, water service will be terminated by the CITY. The CITY assesses fees for turning water service on and off. Special charges may apply for weekend services. The CITY will send past due balances to a third party collections agency after 60 days, and collections fees will be applied to the past due balance. All outstanding water service balances are subject to a lien by the CITY. The CITY shall terminate water service if a leak is causing excessive use of potable water or presents a risk to the water system. CUSTOMER is responsible for contacting the CITY if there are concerns about the condition or accuracy of the water meter. The CITY can be reached at 503-647-5555 or info@northplains.org. For emergency situations call 9-1-1. CUSTOMER and CITY agree the application and agreement may only be amended in writing. This agreement is only for the service address stated above. A separate agreement is required for each water service account. If CUSTOMER should experience a water leak, the CUSTOMER may request a one-time adjustment to the water service bill, when proof of a leak repair is presented to the CITY. CUSTOMER and the owner of the property are responsible for maintenance of equipment and water pipes (including irrigation systems) located between the meter and any the structure on the property. The CITY shall service the water main and pipes connecting it to the water system.
